The Investigator annulled the decision of the OPW. She remitted the matter back to the OPW for fresh consideration.

The Senior Investigator annulled the decision of the NBCMSO. He directed it to make a new decision on the FOI request in accordance with the provisions of the FOI Act.

The Senior Investigator affirmed the Department's decision to refuse access to the record sought under section 42(f) of the Act.

The Investigator affirmed the decision of the HPSC. She found that it was not required to create a record pursuant to section 17(4) of the Act in order to provide the information sought and that it was justified in refusing the request under section 15(1)(a) on the grounds that the record sought does not exist.

The Senior Investigator varied the CSSO's decision. While he found that it was justified in refusing access, under section 42(f), to the information relating to risks 4 and 6 to 10 contained in the Risk Register, he found that it was not justified in refusing access to the remainder of the record under the other exemptions cited and directed its release, subject to the redaction of the names of any specific software/systems used by the CSSO.
